Terms of Service

Welcome to The Space PT LLC. These Terms of Service ("Terms") govern your use of our services. By using our services, you agree to these Terms. Please read them carefully.

1. Overview

The Space PT LLC provides personalized physical therapy services, including but not limited to orthopedic rehabilitation, manual therapy, and sports injury recovery. Our services are tailored to help patients recover mobility, manage pain, and return to their daily activities.

These Terms apply to all users of our services, including patients, website visitors, and any other persons or entities interacting with us.

2. Services Offered
The Space PT LLC provides the following services:

Orthopedic Rehabilitation
Manual Therapy
Sports Injury Recovery
Physical Therapy Consultation and Evaluation
Additional services may be offered based on specific patient needs and availability. All services are subject to the terms laid out herein.

3. Use of Services
By using our services, you agree that:

You are at least 18 years of age or have the consent of a parent or guardian to use our services.
You provide accurate, complete, and current information when requested.
You will comply with all applicable local, state, and federal laws when using our services.
The services provided are intended for personal use and should not be resold or redistributed in any form.

4. Insurance Coverage
In-Network and Out-of-Network Insurance
In-Network Insurance: We are an in-network provider for Medicare, Tricare Select, and various other insurance plans. When using in-network insurance, you will generally pay lower out-of-pocket costs, as the fees for services are pre-negotiated with your insurance provider. Co-pays, coinsurance, and deductibles may still apply based on your specific plan.
Out-of-Network Insurance: If you are using out-of-network insurance (e.g., Tricare Prime without a referral or other non-network insurance plans), a higher portion of the service costs may fall on you. Out-of-network coverage typically involves higher out-of-pocket costs, which may include deductibles, co-pays, and percentages of the billed amount.Example: With Tricare Prime, your co-pay for in-network services is generally $30. For out-of-network services, you may pay a percentage (e.g., 20%) of the total bill, which could be as high as $120 per session.
It is your responsibility to understand your insurance coverage, including any limitations on the number of physical therapy visits, co-pay obligations, and the requirement for a referral. Please contact your insurance provider for confirmation.
Co-pays and Deductibles
Co-pays and deductibles are determined by your insurance plan. Payment of co-pays is required at the time of service. Deductibles and any unpaid portions of services billed through insurance will be invoiced to you based on your insurance company’s explanation of benefits (EOB).

Failure to pay co-pays or deductibles promptly may result in a suspension of services until outstanding balances are cleared.

5. Cash Payments and Pricing
For patients without insurance or those choosing to pay out of pocket, we offer cash payment options. The cost of a cash session is $100, payable at the time of service. You may use FSA (Flexible Spending Accounts) or HSA (Health Savings Accounts) to pay for physical therapy services, as allowed by applicable tax laws.

6. Appointment Scheduling and Cancellation Policy
Scheduling Appointments
Appointments are scheduled on a first-come, first-served basis. We recommend scheduling your appointments at least 2 weeks in advance to ensure availability, as Dr. Razek’s schedule fills up quickly due to high demand.

Cancellations and Rescheduling
Cancellations: If you need to cancel your appointment, we require at least 48 hours’ notice. Cancellations made with less than 48 hours’ notice will incur a cancellation fee of $50, except in the case of emergencies or unavoidable circumstances.
No-Show Policy: If you fail to attend your scheduled appointment without notice, you will be charged the full cost of the session.  Due to high demand and limited availability if more than one No-Show occurs, you will be asked to seek treatment with another provider.
Rescheduling: You may reschedule your appointment without penalty if done at least 48 hours in advance. We will do our best to accommodate your new appointment time.

7. Refunds
Due to the nature of physical therapy services, we do not offer refunds for services already rendered. If you are unsatisfied with the service provided, please contact us directly, and we will work with you to address your concerns.

8. Patient Responsibilities
As a patient of The Space PT LLC, you agree to:

Provide accurate and current medical information, including any relevant medical history and current medications.
Follow the treatment plan prescribed by Dr. Aaron Razak, including home exercises and recommendations provided during sessions.
Attend scheduled appointments and comply with the cancellation policy outlined above.
Failure to adhere to the treatment plan or attend scheduled appointments may result in delayed recovery or suboptimal results.

9. Privacy and Confidentiality
We respect your privacy and are committed to protecting your personal health information (PHI) in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Your personal and medical information will only be shared with your insurance provider and healthcare team as necessary for billing and treatment purposes.

10. Liability
While we strive to provide the highest quality care, The Space PT LLC shall not be held liable for any adverse outcomes or injuries resulting from the misuse of therapy techniques outside of prescribed sessions. Results of physical therapy vary by patient, and there is no guarantee of specific outcomes. You assume full responsibility for following the prescribed treatment plan and for your actions outside of treatment.

11. Modifications to Terms
We reserve the right to modify these Terms at any time. Any changes to these Terms will be posted on our website, and by continuing to use our services, you agree to the revised Terms. It is your responsibility to review these Terms periodically for any updates.

12. Governing Law
These Terms are governed by and construed in accordance with the laws of the state of Colorado. Any disputes arising from these Terms or our services shall be resolved in the courts located within Colorado Springs, Colorado.
